Parent Survey

This survey asks for your opinions about your child’s school. It will take about 5-10 minutes to finish. It uses a 5-point scale, with 1 meaning you “don’t agree at all” with the sentence, and 5 meaning you “agree completely.” Choose one number for each sentence. (Choose the X if you don’t know or have no opinion.)  There is no right answer. Please respond honestly.
